Community Teams

The community teams work with adults aged 18 and above who have a learning disability. Anyone can make a referral to CWP specialist learning disability services. For example: a person with a learning disability, a carer, social worker, residential care staff, day service staff, GP.

Hospital Health Passport (Adults)

A health passport is a document about you.

It tells staff important information about your health needs but also information about how you communicate with other people and the things you like and dislike.

You should take your health passport with you each time you go to hospital.
